OPENING ACCESS TO COVID VACCINE
As of March 29, the state will make covid vaccine available to all adult Ohio residents.
Those 18 years of age and older will have access to both the Moderna or the Johnson and Johnson vaccine.
*Though they are not adults, those 16 years of age and older are able to take the Pfizer vaccine.
Please check with your vaccine provider to know which vaccine will be available to you.
WHERE TO GO FOR VACCINE
Wolstein Center at Cleveland State University
The Federal Emergency Management Association (FEMA) and the Ohio Department of Health have partnered with Cuyahoga County and the City of Cleveland to open the Wolstein Center at Cleveland State University as a mass vaccination site. This operation began March 16 and will continue through the first week of May.
Vaccines are being offered every day, including weekends, from 8 a.m. to 8 p.m.
Registration is necessary and is open to all Ohio residents of age.

To register by phone: 1.833.427.5634
Phone numbers, street addresses and website locations are shown for each provider.
IF YOU ARE ON THE CCBH WAITING LIST
Due to the combination of the state making vaccine available to all adults, the pending increase in supply that is due in April and the presence of the Wolstein Center event, we will suspend our pre-registration process effective March 25, 2021.
We will continue to hold 2-3 large clinics each week, opening spots for those who are currently on our waiting list.
At this time, we do not know how long it will take for us to move through our entire list.
To reduce the risk of contracting covid while waiting, we recommend registering for the Wolstein Center event or contacting a local provider from the list below.
IF YOU HAVE ALREADY RECEIVED VACCINE AND NEED TO BE TAKEN OFF OF THE CCBH LIST
IF YOU ARE COMING TO A CCBH VACCINATION SITE
You will receive two notifications when clinics open up:
For improved functionality, we would recommend you register for a vaccination time slot using your computer instead of your phone.
After you click on the link to register, you will receive a second email message that will have the date and time of your appointment.
You must completely fill out the form and bring it with you when you come to receive your vaccination.
Filling out this form DOES NOT mean that you are automatically registered for a vaccination appointment.
You must wait to be notified to register for an open slot at one of our clinics.
Please bring identification with you when receiving your vaccine.
