CCBH conducts a Swimming Pool Program under the authority of Ohio Revised Code Chapter 3749.

All pools are required to meet the minimum requirements in the Ohio Public Swimming Pool, Spa, and Special Use Pool Rules. These rules are found in Ohio Administrative Code Chapter 3701-31.

A license is required for any pool or spa serving four families or more, which includes apartments, hotels, condominiums, homeowner associations, health clubs, as well as municipal facilities.

Equipment Replacement Notification Form

This form needs to be submitted to the Ohio Department of Health when pumps, filters, types of disinfection, disinfection feeders, or safety vacuum release systems are replaced with a make or model that is different than the current piece of equipment.

Injury Incident Form

An incident is defined as an injury, drowning, near-drowning or suction entrapment occuring at a pool or spa that results in death or requires resuscitation or transfer and admission to a hospital.

Should a reportable incident occur, complete the form and submit via email to mmementowski@ccbh.net

Basic Pool Operations (BPO) – 2026 deadline has passed

License Application

Applications for all previously licensed pools and spas are sent out during the first week of April.

The deadline to return the application without a late fee is April 30.

When renewing a license to operate a swimming pool, state code requires local health departments to charge a late fee for any application/fee postmarked after April 30.

Plan Review Application

This form needs to be submitted to the Ohio Department of Health prior to the construction of any new public swimming pool, or prior to a substantial alteration of an existing public swimming pool, such as the replacement of barrier fencing.
