Animal Bite / Rabies Exposure Report Form – Please complete this form to report an animal bite or potential rabies exposure. Please fax this form to the Cuyahoga County Board of Health at 216-676-1316 within 24 hours of the incident as required by Ohio Administrative Code. For incidents that occur in the City of Cleveland, please fax this form to 216-348-7359 or contact them by phone at 216-664-3069. For incidents that occur in the City of Shaker Heights, please fax this form to 216-491-8829 or contact them by phone at 216-491-1490. Animals must be confined for a minimum of 10 days from the date of the bite.
CCBH Rabies Control Regulations – These local regulations were last adopted on August 2, 2009 and incorporate the required rabies vaccination of any dog, cat or ferret residing in the CCBH health jurisdiction.
