The Cuyahoga County Board of Health conducts inspections for Type B (6 or less children) home day care providers who receive assistance from the U.S. Department of Agriculture. The inspections are required by the Ohio Department of Education – Division of Child Nutrition Services. Inspection reports must be completed by the local health department and fire department on an annual basis to remain in the program.
You can click on this link to download a Residential Child Care Inspection Application. Fill out the application and forward it, along with the appropriate fee, to the Cuyahoga County Board of Health. Upon receiving the application and fee, a sanitarian will contact the provider to schedule an appointment to conduct an inspection. A comprehensive home health inspection will be completed, and the provider will be given any relevant public health information on childcare food safety, disease prevention, and other services. The provider will also be given a copy of the report form after the inspection is completed.
