Immunization Action Plan for pediatric & family practicesThe goal of the Immunization Action Plan is to increase immunization rates of children under age two. It is a collaborative effort between the Cuyahoga County Board of Health, the Cleveland Department of Public Health, Northeast Ohio Neighborhood Centers, and MetroHealth Women, Infants, and Children (WIC). 90% Club Award
This award is presented to those providers that achieve an
immunization coverage rate of 90% or higher for children by the age of 24 months for the
vaccination series: 4 DTaP, 3 Polio, 1 MMR, 3 HIB, 3 Hepatitis B and 1 Varicella (431331).
CCBH would like to recognize the providers in Cuyahoga County who
achieved an up-to-date immunization rate of 90% or greater for 2008:

*Future Generations Pediatrics
*Preferred Pediatrics
Improvement Award
This award is presented to providers that have shown great
improvement in their immunization coverage rates for children by 24 months of age from one year’s
assessment to the next. Specifically, they meet the following criteria:
1. An immunization coverage rate of 70% or higher for children by 24
months of age for the 431331 series in 2008
2. A minimum improvement margin of 20% from the immunization coverage
assessed 431331 rate from the previous assessment (2006 or 2007) to the 2008 assessment 431331
immunization coverage rate.

improved their immunization rates from 2006 or 2007 to 2008:
