Immunization Action Plan for physician offices

The goal of the Immunization Action Plan is to increase immunization rates of children under age two.  It is a collaborative effort between the Cuyahoga County Board of Health, the Cleveland Department of Public Health, Northeast Ohio Neighborhood Centers, and MetroHealth Women, Infants, and Children (WIC).

Program Services

  • Service delivery including providing immunizations to uninsured/underinsured children through public health immunization clinics
  • Reminder phone calls to parents when children are behind in their immunizations.  
  • Assessment and education services  are provided free of charge to pediatric and family practice provider utilizing the Maximizing Office Based Immunization (MOBI ) currriculum, which emphasizes implementing proven strategies to improve rates within provider practices.
