Programs for the Aging Population

Title:  Home Injury Prevention Program "H.I.P.P."    

The Cuyahoga County Board of Health (CCBH) has established a home injury prevention program, which is made possible with funding from the Western Reserve Area Agency on Aging (WRAAA).   The goal of the program is to decrease or eliminate the risk of unintentional falls within the home for each client.  The program is designed to complete home assessments for seniors who meet specified eligibility guidelines.   The qualifications include risk factors that put each person at a specific risk level.  Each client will be assessed on an individual basis when calling the intake line.  Once the client is determined to be eligible, a case worker is assigned.  The case worker will call the client and set up a home visit.  The number to call to see if you qualify is 216-201-2001, extension 1519.  OR Call 216-201-2000 and ask for H.I.P.P.  In addition to home visits, educational sessions are offered county wide for agencies, home care workers and senior centers on a per request basis.   Please call 216-201-2001 ext. 1538 to set up an educational talk at your agency.  Please click here  if you would like a printable tip sheet on how to reduce the risk of falling in your home.

Title:  Matter of Balance

Again partnering with Western Reserve Area Agency on Aging, we were awarded a grant from Administration on Aging via the Ohio Department of Aging to offer The Matter of Balance Program (M.O.B.).  This program is an evidence based program to specifically reduce the fear of falling and improve activity level among senior adults in community dwellings. The program consist of eight two hour classes presented by trained coaches using detailed manuals and two instructional videos.  A Matter of Balance Lay Leader model has been recognized for Innovation and Quality in Healthcare and Aging in 2006 by the American Society on Aging, The program was originally developed via a grant with AoA by Boston University and now is currently being implemented across the county.  This program is made possible with funding from The Western Reserve Area Agency on Aging.  If you would like to view a power point presentation describing the history and goals of this program in detail,        please click here.   Call 216-201-2001 ext. 1209 or 1206 for more information.
