Community Based Nutrition Services

PROGRAM DESCRIPTION:  The Cuyahoga County Board of Health (CCBH) community based nutrition services program was created to provide nutrition care for children with special needs and their caregivers.  Registered dietitians at CCBH receive referrals from the Cuyahoga County Board of Mental Retardation and Developmental Disabilities and the Bureau for Children with Medical Handicaps and provide home and center based nutrition services to children and families.  Families can receive a nutrition assessment for their child, as well as information, counseling, teaching, monitoring, and referrals to other providers.  CCBH Community Based dietitians also provide nutrition trainings to outreach staff from various community agencies working with nutritionally at-risk populations.

CCBH community based dietitians are available to provide public nutrition forums and worksite nutrition seminars upon request.

OBJECTIVE:  The CCBH community based nutrition services program was created to provide up-to-date, scientifically based nutrition information to the residents of Cuyahoga County.  The nutritionally at-risk population was targeted in an effort to improve clients overall health and reduce morbidity and mortality.

COMMUNITY PARTNERS:  Cuyahoga County Board of Mental Retardation and Developmental Disabilities, Help Me Grow of Cuyahoga County, and the Bureau for Children with Medical Handicaps.
