Child and Family Health Services Grant Program

Child & Family Health Services Grant

This program is designed as an organized community effort to improve the health status of women and children in Cuyahoga County, including assessment and monitoring of maternal and child health indicators at the individual community or neighborhood level.  The program also provides funding to partner agencies to address child and adolescent health needs, as well as the needs of pregnant women and teens at risk for a poor birth outcome. The grant program encompasses three different component areas, with specific activities being carried out by CCBH and six subgrantees as follows:

Community Assessment & Planning Component
• Partnership building and consortium development
• Data collection and analysis
• Intervention development and best practices research
• Evaluation of programmatic efforts and quality assurance activities
• Collaboration with the Cuyahoga County Child Fatality Review Program to develop strategies to prevent child deaths
• Collaboration with other county agencies to assure systems of quality health care in Cuyahoga County

Child & Adolescent Health
• Provision of direct health care services (through MetroHealth)
• Provision of assistance to families and children to gain access to Medicaid
• Provision of care coordination services for at-risk children
• Provision of nutrition assessment, counseling, and education for overweight children and adolescents
• Implementation of a focused community education campaign regarding safe infant sleep practices
• Provision of technical assistance to school districts in establishing federally mandated school wellness policies aimed at reducing the number of children who are overweight
• Provision of assessment, counseling, or education for child and adolescent health clients with identified social/emotional risk factors

Perinatal Health
• Provision of direct health care services (through MetroHealth)
• Provision of assistance to pregnant women to gain access to Medicaid
• Provision of education and referral services to pregnant women identified as being at risk for a preterm birth
• Provision of care coordination services for high-risk pregnant clients
• Provision of services aimed at ensuring the social/emotional health needs of pregnant women are met
